[Ada] Couple of small cleanups for Cloned_Subtype

No functional changes.

gcc/ada/

	* exp_util.adb (Make_Subtype_From_Expr): Do not set field to Empty.
	* sem_util.adb (Visit_Itype): Remove ??? comment.
