made a function getKeyCounter() to automatically increment a counter
and make sure it never equals zero.  For use with private keys
and ICP query headers.

Fixed setting reqnum on outgoing ICP requests.  Now either set to
private key number or a new non-zero number.

oops, already have data->entry->mem_obj->fd_of_first_client, don't need
storePendingFirstFD()
