usb: xhci: drop xhci-caps.h dependence on xhci-ext-caps.h

Drop the dependency of xhci-caps.h on xhci-ext-caps.h by eliminating 2
instances where macros in xhci-caps.h were redefined from xhci-ext-caps.h.
