Simplify ASGI concurrency (#248)

* Drop  keyword argument

* Improve docstrings for WSGIDispatch and ASGIDispatch

* Add docs for fine grained WSGI/ASGI control

* Simplify concurrency handling in ASGIDispatch

* Variable renaming

* Fix missing nonlocal declaration

* Split nonlocal onto multiple lines
