* texinfo.tex (\uref): implement optional third arg.
* texinfo.tex (\dosubind): Include index entry in third arg to
\entry instead of writing as bogus fourth arg.

* texinfo.tex (\setemergencystretch): Increase somewhat.
* texinfo.tex (\putwordof): rename from \putwordOf.
