redo outlines

- removes original outline removal (hah)
- replaces it with an explicit `outline: 0` on `.btn`
- instead of replicating `:hover` for `:focus`, uses custom and themed `box-shadow` for an "outline"
- not mapped to the `$enable-shadows` variable because accessibility
