ftests: Fix failure cause when multiple tests don't pass

When multiple tests don't pass, the last failure_cause
recorded was the one being displayed.  Append the
failure_cause to the failed_tests[] and skipped_tests[]
arrays, so that each failure is properly saved and
reported.
