Refactor CSS variables and design tokens (#41922)

- Rename --bs-color-* to --bs-fg-* for foreground colors
- Simplify font-size variables (remove clamp for xs/sm/md)
- Remove deprecated secondary/tertiary color variables
- Clean up commented-out legacy variable declarations
