Normalize the th element

Normalize the `th` element. I have specifically observed that `th`
elements sometimes show content vertically aligned in the middle. `th`
elements are used for all columns, so they should be normalized to be
vertically aligned to the top

Set maxLineLength option to 800 for htmlmin

Gmail automatically adds linebreaks if lines are too long and doesn't care about code being split by newlines at invalid HTML split-points. This property prevents this to happen.

Set maxLineLength for htmlmin to 78 char

As stated here https://www.emailonacid.com/blog/article/email-development/line-length-in-html-email, setting max line length to 78 char prevents some email clients in some cases to truncate, or disastrously wrap lines which may cause problems in html rendering.

Remove margins from anchor elements

There is a bug in Outlook 2016 (and possibly other versions) that leads
to a `p` element losing its `margin` if there is an `a` element inside
the `p`, and the `a` element has css `margin` specified. `a` elements
are by default inline and without margins, so it shouldn't cause any
problems to solved the issue by just not specifying `margin` for `a`
elements.

!important styles are ignored by Outlook when they are inlined. There
are no other rules that would override the width of the expanded
buttons, so it is not necessary to have it declared as !important.
Removing !important means that the style will be inlined as
style="width: 100%" instead of style="width: 100% !important". The
latter is ignored by Outlook, while the first one works and makes the
