From now on, autoscan files must always map a macro name to a
word: there is no `default' macro for autoscan.
* bin/autoscan.in (&init_tables): Reject entries with no macro at
all.
* lib/autoscan/functions, lib/autoscan/headers: Make the macro
explicit.
