mod_dbd backports:
  (1) Key connection pools to virtualhosts (niq, minfrin; reviewed us + wrowe)
  (2) share per-request database handles across subrequests (chrisd; reviewed
      niq + rpluem).  Including fixing compiler warning (rpluem).
